Do you have a participant who doesn’t quite fit the SIL model but requires more support than just ILO?
HV Care offers a solution with our Assisted Rental Services.
This service gives participants the choice and control to let us assist them to find a rental property, support with setting up their home and offer the daily drop-in support from our staff.

HV Care’s Community Nursing Care services are designed to provide specialised nursing care within the comfort of your own home. We offer nursing supports with continence assessments, Chronic disease management, medication management & wound care.
Our team delivers compassionate and expert care tailored to the unique requirements of each participant. By bringing high-quality nursing services directly to the community, we aim to enhance the overall well-being and health of our clients while promoting independence and continuity of care.
ILO or Individualised Living Option is an accommodation option for participants that require a minimum of 6 hours of support (which can be formal or informal) per day but don’t require 24/7 support such as SIL (supported independent living). As the support provider we offer the formal support daily however ILO doesn’t include securing the home of your choice nor do we offer assisting with living expenses.
Our services are dedicated to assisting individuals living in their own home with their daily support needs. We want to promote their independence by developing and re-establishing daily living skills and self-care strategies. Our comprehensive approach includes providing personal, social, and domestic care.
Supported Independent Living (SIL) is a type of housing and support service that helps people with disabilities who require 24/7 care. We offer SIL services in a variety of settings, including shared homes, apartments, and townhouses.
Our SIL services are designed to promote self-sufficiency and independence. We work with each participant to develop a personalised support plan that meets their individual needs and goals. Our support staff can assist with a variety of tasks, such as activities of daily living, medication management, transportation, budgeting and financial management, social & community engagement.
A participant will generally be able to access funding through the NDIS for transport assistance if the participant cannot use public transport without substantial difficulty due to their disability. We can offer transport options as well as travel training to participants that are ready to take the next step into their independence

All services are subject to a minimum charge of one hour. Operating hours are Monday to Friday, from 7:00 am to 7:00 pm, excluding public holidays. Fees are reviewed annually at the start of each financial year and may be adjusted at any time.
Participants will receive an invoice at the end of each month. Invoices will be sent via email or post, and payments can be made through direct debit or using the payment options provided on the invoice. Late payments or non-payment of fees will be handled according to the HV CARE Fee and Debtor Management Policy and Procedure. If participants experience financial hardship or a change in financial circumstances, they may apply for a fee reduction or waiver by contacting our service team for assistance.
A minimum of 48 hours’ notice is required to cancel or temporarily suspend your service. If sufficient notice is not provided, you may be charged up to the full cost of the service.
